Community Support Worker - Galway

We are seeking Community Support Workers - New Directions and Outreach for Day Services based in Galway City.

As part of your role, you will :

• Empower Service Users to live independent lives in their home and the community.
• Support the development and delivery of the person-centred plan
• Develop community partnerships to foster active citizenship in line with service user goals and wishes.
• Transport service users to access the community.
• Be responsible for Key Worker duties i.e. weekly administration, advocacy, liaising with families and develop and implement person-centred plans.
