Overview

CREGG is delighted to be recruiting a Staff Nurse to join a well-known nursing home in Gort.

In this role the candidate will report to the Clinical Nurse Manager and will be responsible for providing and maintaining the highest standards of person-centered care to Residents at The Nursing Home.

The role holder will apply their professional clinical expertise in line with governing standards and best practice.

Be responsible for ensure appropriate risk management to ensure the safety, health and welfare of residents.

Responsibilities

Conduct person centered assessments of the needs of Residents according to national regulations, HIQA standards, professional guidelines and The Nursing Home policies and procedures.

Involve the Residents as partners in developing care plans to meet their needs.

Assessment, planning, delivery and evaluation of care to Residents assigned to their team.

Responding appropriately and in a timely manner where a Resident's condition deteriorates.

Ensuring that referrals to allied healthcare professionals for individual residents are made in timely manner in accordance with each Residents changing needs.

Liaison with any allied healthcare professional involved in a resident's care to ensure a collaborative approach to care planning; safe handover of information and continuity of care for residents.

Maintenance of current, factual and accurate assessments and care plans for residents.

Daily monitoring and review of Residents care and condition and documenting of same in residents' care plans and daily notes.

Maintenance of competence in all areas of assessment and care planning for residents.

Initial and ongoing risk assessment of residents' disease diagnosis, care, treatments and condition and documenting of same.

Ensuring that equipment in use for caring for residents is in good working order and reporting any concerns to the Person in Charge.

This includes all manual handling equipment and aids; mattresses, beds and bedrails.

Checking that fire exits in their area are free from obstruction.

Communicate with chef / kitchen manager and kitchen staff to ensure the nutrition and hydration needs of residents are met according to their assessed needs and in compliance with the national nursing home regulations and standards.

Provision of accurate and current information in all handovers of care situations.

Compliance with the organizations policies and procedures specific to their role.

Use of evidence based approach to all decision making related to the assessment, planning, provision and evaluation of care to Residents.

Delegation to junior colleagues only in the best interests of Residents and based on the knowledge, experience and skills of junior colleagues.

Provision of supervision to healthcare assistants in the delivery of care to residents.

Reporting to the Person in Charge on any aspects of concern for the quality and safety of care to residents.

Compliance with safe medication practice as outlined by NMBI guidance and The Nursing Home medication management policy and procedures.

Cleaning and maintenance of equipment for use in caring for Residents in accordance with policies and procedures and specific manufacturer's instructions.

Reporting all incidents, accidents, near misses and property damage in accordance with The Nursing Home's risk management policy and procedures.

Involvement in risk management and quality improvement initiatives as determined by the clinical governance committee.

Liaising with the Person in Charge where direction and / or advice is needed with care planning for individual residents.

Conduct assessments of risks individual to each Resident as part of the preadmission, admission and comprehensive assessment processes.

Identify individual risks to the safety, health and welfare of Residents in The Nursing Home.

Record and report all incidents / accidents; near misses and medication errors noted in accordance with the incident management procedures for The Nursing Home.

Adhere to national standards for the prevention and management of infections.

Quality Improvement.

Identify review / evaluation schedule of care for individual resident residents and conduct and record same in the resident's progress notes.

Act as a 'Link Nurse' or resource person for an identified topic of specialty practice e.g. Falls Prevention and Management, Infection Control and so on.

Maintain a current up-to-date knowledge, experience and practice, based on the best available evidence related to speciality topic.

Maintain own professional development by attending relevant training and educational sessions and conferences relative to specialty topic.

To be a leader for change practice initiatives relating to the specialty topic, by actively engaging staff in the implementation of relevant policies and procedures.

Conduct audits of structures, process and outcomes in accordance with the annual audit programme and as delegated by the Person in Charge.

Maintain timely and accurate documentation related to care, health and safety; risk management and quality in accordance with national standards; regulations; local policies and professional guidance.

Qualifications and Experience

Be registered Nurse with NMBI.

Experience of Working with Older People.

Evidence of Continuing Professional Development in the last two years.
